The utilisation of hospital services is increasing due to ageing and the increasing incidence and complexity of chronic diseases.For the benefit of health service consumers and optimising the use of resources,new models of care to assess and manage patients in the community rather than in hospitals are required.This presentation outlines three innovative models of care which have been instituted to minimise use of emergency department and in-hospital beds by general medical and aged care patients.One other model significantly reduces length of stay in hospital.Diagnoses managed include infections,falls,delirium.Outcomes include rapid assessment(2-48 hours depending on the model) ,avoiding 90% of admissions for those referred ,and positive feedback from consumers and partners.Experience,communication and teamwork are the requirements for these models to succeed.
